NEW PATIENT’S IDENTIFICATION
On your first visit to our office, you will be required to bring your insurance card (if applicable), AB Health Card and a photo ID. This will help us ensure the true identity of the patients that come to our office.
Our Office collects information such as address, telephone number, mobile number, and email address collectively known as contact information. These information is required for billing, confirming your appointment, sending important information such as copies of referrals, clinic announcement, among other things.
Our office sends text message and/or email reminders 2 weeks, 1 week, and 2 days prior to your appointment. We also give reminder calls 2 business days prior to your appointment, unless your appointment was already confirmed through email or text. However, please understand that it is your responsibility to keep track of your appointments.
We understand that there are times when you must miss an appointment due to emergencies or obligations for work or family. However, when you do not call to cancel an appointment, you may be preventing another patient from getting a much needed treatment. We require a minimum of two business days’ notice when cancelling, or re-booking your appointment.
If insufficient time is given to make changes to your appointment, a $50 fee will be charged and this will not be covered by your insurance.
Our office takes profile pictures of patients for identification purposes. We also take intra-oral photographs for reasons such as: comparing before and after treatment, for sending predetermination to your insurance, etc.
PREDETERMINATION OF TREATMENT
Most insurance companies require authorization for certain procedures like crowns, dentures, bridges, etc., before we can begin with the treatment. Our office prefers to have a hard copy of this approval to ensure that part of the cost of the treatment will be covered by your insurance. However, you can choose to start with the treatment before the approval is received. In which case, if your insurance refuse to pay for the treatment, you will be responsible to pay for the fees. Please understand that the approval for the treatment DOES NOT guarantee payment if your coverage gets terminated, or if your annual maximum is reached before the treatment is finished.
DIRECT BILLING & ASSIGNMENT OF BENEFITS
Assignment of Benefits means that when the dental office sends the bill to your insurance, your insurance will send the payment directly to the dental provider (dental office), whether by cheque or electronic fund transfer. This will eliminate the hassle of the patient doing all the paperwork, as well as the burden of having to pay the total cost of the treatment.
Our office extends the courtesy of providing this service, as long as your insurance allows it. But because of the Privacy Act, dental offices have limited access to your insurance information. It is therefore your responsibility to know the details of your dental coverage (including your annual maximum, benefit year, percentage of coverage, frequencies, and other limitations). Your knowledge of your dental plan will help you utilize your benefit to the maximum.
If you would like to avail of this service, we will be happy to take care of all the paperwork necessary to claim the payment from your insurance. However, a credit card number is required to be put on your file. Your copay will be charged on the credit card once we confirm or receive the payment from your insurance. If you prefer NOT to leave a credit card information, you will be required to pay the full cost of the dental service(s) rendered at the time of your appointment.
Any claim not paid by your insurance within 60 days will be automatically charged to your credit card.
The same financial policy is followed if you have a dual (or more) coverage. We will submit the paperwork to your 2nd dental plan after we receive the payment from your primary plan. Any remaining amount not covered by both plans will be charged on your credit card on file.
For your convenience, we accept VISA/MASTERCARD/AMERICAN EXPRESS/ INTERAC/ CASH. We do not accept personal cheques.
